What to Bring on Your First Visit
- State drivers license
- insurance card or information
- all health providers contact information
- current medication list
- completed patient forms (forms will be emailed to you).
- doctor’s referral, if required
- medical records
- X-ray
We understand that emergencies are unavoidable. Please call our office at least 24 hours in advance if you are aware that you will not be able to keep your appointment. We will be happy to reschedule you to a more convenient time. This will allow us to better serve you and your fellow patients. This will also allow us to schedule “same-day” emergent patients.
Please arrive 15 minutes before your scheduled appointment so that we can ensure all paperwork is completed. This will allow us to maintain our schedule throughout the day and avoid any delays. Your first visit can be lengthy and we want to ensure Dr. Kennedy Legel has plenty of time on your visit.
